Who Do We Work With?
We work with adults 18 and over at risk for/experiencing homelessness with substance abuse OR substance abuse and mental health diagnoses.
This means anyone sleeping from couch to couch or in someones guest room. Anyone down on their luck and everything in between.
How Can We Help?
Project Safe Horizon provides case management and care coordination services indcluding but not limited to intensive outpatient treatment, recovery support services, and benefit engagement and enrollment services.
If you need help applying for veterans benefits, SNAP benefits, medicaid benefits, or health insurance. Or if you need counseling, treatment, or help finding housing please let us know!

What this means for you is immediate assistance to get you back on your feet. You don't have to do it alone and our services are 100% free of cost.
If you or anyone you know needs assistance locating primary healthcare, enrollment in vocational training, assistance with parenting skills, treatment, counseling, transportation, or job placement we are able and willing to help.
